A Year of Whole-Person Care at CITM
Your baseline visit. Your initial evaluation provides you with a true baseline evaluation, complete medical history, ongoing lab results, medications, lifestyle habits, and any family health patterns that are worth noting. This is the history that is being added to during subsequent visits.
Three months in. Early check-in reviews what is going on, before things are registered as a done deal. Changes in dietary, medication, or complementary treatments occur here when there are issues that do not align.
Six months in. Chronic conditions (if any) are re-evaluated against actual data and not general impressions. This is also a natural time to add sick visits or a couple to your regular record, instead of viewing them as separate, stand-alone occurrences.
Twelve months in. This year’s lab results and overall picture are contrasted with those of the previous year during the annual review, a process not easily accomplished by just taking a yearly physical. From this point, the following year’s plan is formulated.
